Integrating Families, Communities and Professionals (IFCAP)

Integrating Families, Communities, and Providers (IFCAP) is a unique partnership between the Florida Institute for Family Involvement (FIFI), families and youth, and state and community partners to begin building a road map that will lead to better services, supports, and resources for children and youth with special health care needs. IFCAP and its partners promotes a system of care for children and youth with special health care needs (CYSHCN) that is:

  • Accessible
  • Family Centered/Driven-Youth Guided
  • Continuous
  • Comprehensive
  • Coordinated
  • Comprehensive
  • Culturally & Linguistically Competent

The IFCAP project promotes the medical home concept and family-professional partnerships through the development of community coalitions in an effort to facilitate community based services and supports for families and their children and youth with special health care needs. Major components of the IFCAP project include:

  • Statewide Taskforce of partners and experts in services to children and youth with special health care needs and their families. The Taskforce, which includes families and youth, offers advice and guidance on the project.
  • Selecting and working intensively with four communities in the development of community partners and approaches to services and supports for CYSHCN and their families; The four, who successfully competed to participate in the IFCAP Project are: Jacksonville, Sarasota, West Palm-Broward, and the Keys.
  • IFCAP Family Involvement Specialists (IFIS), who are parents of CYSHCN, to work within their community with partners; build linkages and foster a family centered approach to services, supports, and resources.
